Terms and Conditions:
- Membership includes discounts on non-surgical services as outlined on our website and in brochures.
- Discounted treatments may only be used by the member and may not be assigned, transferred, or otherwise disposed of by the member.
- Applicants for membership must be at least 18 years old or receive parental consent if under 18.
- A one-time $50 set up fee will be charged at time of sign-up.
- Monthly membership payments shall be made in advance by direct debit from the member’s designated credit/debit account each month from the date specified below. This information will be kept securely on file by Utah Facial Plastics.
- Either party may cancel the membership at any time.
- Utah Facial Plastics reserves the right to review subscriptions periodically. Members will be given at least 30 days’ notice of any changes.
- If monthly payment to UFP is declined, we will give 30 days to update or pay after notifying the member by phone or email. If membership is made current within 15 days, there will be no additional $50 fee to re-assign. After 30 days, the membership will default.
- If Utah Facial Plastics terminates for any reason, they reserve the right to retain any monies received to cover any reasonable fees the member has incurred as a result.
- Banked monies cannot be withdrawn and must be used on aesthetic services with UFP only.
- Discounts may not be combined with any other promotional offer or promotional gift card. Monies can be used toward membership discounts OR specials.
- Monies cannot be used towards surgical procedures or fees at any time.
