Filler

Filler is a very common treatment used to treat under-eye hollowing. As of today, no filler on the market has been FDA-approved to specifically treat under-eye hollowing, although clinical trials are currently in progress. That being said, filler has been used “off-label” for a number of years to treat hollowing underneath the eyes. When performed by an experienced injector who is well trained in facial anatomy, hyaluronic acid filler has proven to be a safe and effective under-eye treatment. Permanent and semi-permanent fillers come with a greater risk of complication in this area. At Utah Facial Plastics, we only treat patients with temporary hyaluronic acid fillers, preferably Restylane, which lasts 9-12 months on average in this area. The results are smooth and natural with less risk of swelling than other injectable products.

Blepharoplasty and Fat Grafting

Surgery is a permanent solution for patients concerned with moderate to advanced under-eye hollowing. There are a variety of surgical options available for patients depending on their needs and concerns. Blepharoplasty (lower lid surgery) can remove both excess skin and fat lax skin while fat grafting is used to “fill in” hollowing by adding fat. Some patients may require a combination both under-eye surgery and fat grafting for full correction. Fat grafting, also known as “fat injections” or fat transfer, is performed in-office under local anesthesia. The donor fat is taken from the patient’s abdomen or thigh area and then injected to the under-eye hollows. Our surgeons transfer more than what is needed as not all the fat cells will survive. During the process, typically 50% of the fat attaches to a blood supply and becomes permanent.

In some cases, the “repositioning” of fat may be required in conjunction with eyelid surgery. This involves taking the fat under the eyes and repositioning it into the hollows to create a smooth junction between the under-eye and cheek areas. This surgery is typically done in an operating room under general anesthesia.
