Who this notice covers
Joint notice. This notice covers three affiliated legal entities: Utah Plastic Surgery, LLC, doing business as Utah Facial Plastics (“Utah Facial Plastics”); UFP Surgery Center LLC, the QUAD A accredited surgical facility (“the Surgery Center”); and Utah Facial Aesthetics LLC, the med spa (“the Med Spa”). Where this notice says “we” or “our practice,” it means all three unless a specific entity is named. You may receive care, and be billed, by more than one of them for a single course of treatment.
These entities participate together in your care and share protected health information with each other as necessary for treatment, payment and health care operations. This is a joint notice: one copy, and one acknowledgment, covers all three. This notice applies to all of our service delivery sites, including our offices in Draper, Layton and Heber City, Utah, and the Surgery Center.
At our practice we value your relationship with us. We understand that your medical and health information is private. We know that respect for that privacy is a critical part of our relationship, and we are committed to protecting the privacy of your protected health information.
We follow strict federal and state laws that require us to maintain confidentiality of your health information. This Notice of Privacy Practices was created to help you understand our legal duties to protect your health information, as well as your rights regarding your health information.
How We Can Use Your Health Information
When you receive care from our practice, we may use your health information for purposes such as providing treatment, processing billing for services, and conducting routine healthcare operations. Below are examples of how we may use and share your health information.
- Treatment. We maintain records of the care and services provided to you. Health care providers rely on these records to deliver high-quality care that meets your needs. Your physician may share your health information with specialists, facilities, or representatives involved in your care.
- Payment. We keep billing records that include payment information and documentation of services provided to you. Your information may be used to obtain payment from your insurance company, verify coverage for your care, or notify them of upcoming services.
- Health care operations. We use health information to continually improve the quality and effectiveness of the health care services we provide. Your health information may be used to conduct essential business operations.
Use of Your Health Information
We may use your health information to:
- Recommend treatment alternatives.
- Inform you about health services and products that may benefit you.
- Share information with third parties who assist with treatment, payment, and healthcare operations. These third parties, known as business associates, are required to follow our privacy policies.
Sharing Your Health Information
In certain situations, we are permitted or required by law to disclose health information without your written authorization, including:
- For public health purposes, such as reporting communicable diseases, work-related illnesses and injuries, deaths, or adverse reactions to drugs or medical devices.
- To protect victims of abuse, neglect, or domestic violence.
- For health oversight activities, such as audits, investigations, and inspections.
- For lawsuits and similar legal proceedings.
- When otherwise required by law.
- When requested by law enforcement, as required by law or court order.
- For workers’ compensation or similar programs if you are injured at work.
- For specialized government functions, such as intelligence or national security.
Authorization Requirement
We will obtain your written authorization before using or disclosing your health information for any purpose not listed above, or as otherwise permitted or required by law. You may revoke this authorization at any time with a written statement.
Our Privacy Responsibilities
As required by law, we are committed to:
- Maintaining the privacy of your health information.
- Providing this notice to explain how we may use and share your health information.
- Following the terms outlined in the current notice.
We reserve the right to update and revise this notice at any time. The most current version will be available on our website at www.utahfacialplastics.com and in our offices.
Your Individual Rights
You have the right to:
- Request restrictions on how we use and share your information, though we are not obligated to agree to these requests.
- Specify an address and phone number for us to use when communicating with you.
- Inspect and request copies of your health information, both medical and billing records, with possible fees.
- Request corrections or additions to your health information, provided these requests are submitted in writing.
- Request an additional copy of this notice.
- Request an accounting of certain disclosures of your health information. Note that this does not include disclosures for treatment, payment, or health care operations, nor any disclosures made before November 13th, 2016.
