Cancelation Policy
- Your deposit, if applicable, may be applied toward any non-surgical treatment or product within one year if you attend your appointment but decide not to proceed with treatment.
- To receive a full refund, cancellations or reschedules must be made at least 48 business hours in advance. Because your appointment time is reserved exclusively for you, deposits will be applied toward a cancellation fee if proper notice is not provided.
- Additional charges may apply based on the length of your appointment.
- Please note, this fee reflects a missed business opportunity rather than a service charge.
Intake Paperwork
All new patient paperwork must be completed online prior to your appointment. You will receive a link via text and email if you are a new patient or an existing patient due for updated paperwork. We will continue to send these reminders until all paperwork is filled out so you have adequate time with your provider at your appointment. They cannot legally see you without all the necessary information so please complete this ahead of time or come 15 minutes early to your appointment for assistance.
