Pre-Appointment Instructions

Thank you for scheduling with Utah Facial Plastics! Please review the following information prior to your appointment.

CANCELLATION POLICY

As a reminder, the $50 or $150 you put down to schedule your appointment (if applicable) can be applied toward any non-surgical treatment or product within one year if you come to your appointment and decide not to move forward with treatment at that time. If you need to cancel or reschedule, we require at least 48 business hours' notice and can refund the full amount. Because we reserve a time especially for you, your deposit will be applied toward a cancellation fee without proper notice. The charge is for a missed business opportunity and not a charge for the service itself.

INTAKE PAPERWORK

All new patient paperwork must be completed online prior to your appointment. You will receive a link via text and email if you are a new patient or an existing patient due for updated paperwork. We will continue to send these reminders until all paperwork is filled out so you have adequate time with your provider at your appointment. They cannot legally see you without all the necessary information so please complete this ahead of time or come 15 minutes early to your appointment for assistance.
