CANCELLATION POLICY
As a reminder, the $50 or $150 you put down to schedule your appointment (if applicable) can be applied toward any non-surgical treatment or product within one year if you come to your appointment and decide not to move forward with treatment at that time. If you need to cancel or reschedule, we require 48 business hours' notice and can refund the full amount. Otherwise, that amount is then applied toward a cancellation fee.Â
INTAKE PAPERWORK FOR ALL PATIENTSÂ
Due to an update to our software, new paperwork must now be completed ONLINE for BOTH NEW AND EXISTING patients. That is only for existing patients who originally filled out their paperwork prior to April of 2022.Â
If you receive a link (available for 72 hours only) stating you have incomplete forms, please update them through the link on your mobile phone. Existing patients CAN SKIP THE DEMOGRAPHICS section, but we ask you at least review everything. New patients must fill out the demographics, medical history, and then just initial the 2 consent forms. We will continue to send these reminders until all paperwork is filled out, so you have adequate time with your provider at your appointment. They cannot legally see you without all the necessary information so please complete this ahead of time or come 10 minutes early to your appointment for assistance. Â
