Wellness Program Pricing & Packages
Medical Weight Loss
Pricing
A streamlined program for patients focused on safe, effective, and long-term weight loss. Micro-dosing options for inflammation reduction are also available for qualifying patients.
New Patients & First Month of Treatment
Initial Consultation: $100
- This includes:
- A weight assessment
- Comprehensive evaluation
- Personalized treatment plan
- A review of labs (not required for assessment and treatment, but recommended)
First Month of Medication: $150-$200
- Semaglutide: $150
- Dose, prescription and pharmacy dependant
- Tirzepetide: $200
- Dose, prescription and pharmacy dependant
Monthly Pricing & Treatment Plan:
Applies to all subsequent months of treatment
- Semaglutide: $300
- Tirzepetide: $400 (Microdose: $300)
Pricing may increase with higher dosage levels.
This includes:
- Ongoing provider oversight and support
- Prescription weight loss medication shipped directly to your home
- Lab ordering and review
- Follow-ups as needed at no additional cost

Notice to Medicare Patients (Age 65+)
Due to Medicare regulations, Utah Facial Plastics is unable to provide hormone therapy or medical weight loss services to patients who are enrolled in Medicare. However, we are happy to offer our full range of other wellness and aesthetic treatments. If you have questions about eligibility or available services, our team is always happy to help.
Hormone Replacement Therapy Pricing
This program offers ongoing, medically supervised hormone optimization tailored to your specific needs.
New Patients
Initial Consultation: $100
Follow-Up Visits: $100
The first follow-up visit is typically at 6 weeks. While hormone doses are being adjusted, visits occur every 8-12 weeks. Once levels are stable, follow-up visits are generally scheduled every 3-6 months.
Services Include:
- Ongoing medical management
- Follow-up visits (telehealth or in-person)
- Lab ordering and review
- Medication dose adjustments
- Prescription management
- Support services from provider & UFP team
Medications are paid directly by the patient, and filled at the patient's preferred pharmacy or a pharmacy recommended by your provider.

Utah Facial Plastics offers IV Hydration Therapy and Vitamin Injections to support energy, immunity, recovery, and overall wellness. Unlike oral supplements, these treatments deliver nutrients directly into the bloodstream, allowing for up to 90% absorption for more efficient, effective results.
